These terms apply when you use the Nola website or ask Nola for help. By using the site or sending us a request, you agree to them.

What Nola does

Nola provides care management. We help older adults and their families with benefits, paperwork, food, rides, home care and coordination outside the doctor's office.

Clinical services, including clinical intakes and oversight of care plans, are provided by physicians at Nola's affiliated medical practice, which is responsible for medical decisions. Patients keep their own doctors, and our affiliated physicians work alongside them.

Nola's care managers don't diagnose, treat or give medical advice, and Nola doesn't give legal or financial advice. Nothing on this website is medical advice. For health questions, talk to your doctor.

Medicare and costs

Care management services are billed to Medicare by our affiliated medical practice. Like other Medicare Part B services, they may involve a coinsurance, which supplemental coverage or Medicaid may pay. We'll explain expected costs before services begin, and you can stop services at any time.

Not an emergency service

Nola is not an emergency service and doesn't monitor messages around the clock. If someone is in danger or needs urgent medical care, call 911. Please don't use our forms or email for urgent medical issues.

No guarantees about outcomes

We work hard on every request, but many outcomes depend on other people and organizations. We can't guarantee that a benefit, program or application will be approved, that Medicare or another insurer will cover a particular service, that a provider, agency or service will be available, or that any particular result will happen. Decisions made by insurers, agencies, providers and benefits offices are theirs, not ours.

Your responsibilities

• Give us information that is accurate and up to date, as best you know it.
• If you share information about someone else, such as a parent, patient or client, make sure you're allowed to share it with us.
• If you're referring a family, tell us whether they have agreed to be contacted.

Other organizations

We may connect you with providers, agencies, programs and vendors that are separate from Nola. They have their own terms and policies, and we aren't responsible for their services.

The content on this site is general information and is provided "as is." We may update or change it at any time.

Limitation of liability

To the fullest extent the law allows, Nola is not liable for indirect or consequential losses, or for the actions or decisions of third parties, arising from your use of this website or our services.

Privacy

Governing law

These terms are governed by the laws of the State of New York.

Changes to these terms

If we change these terms, we'll post the new version on this page with a new effective date.
