Privacy policy.
Effective October 15, 2026
This policy explains what information Nola collects, why, who sees it, and what you can ask us to do with it.
Who we are
Nola provides care management for older adults and their families in the New York City area. We help with benefits, paperwork, food, rides, home care and the other work that happens outside the doctor's office. Clinical services, including clinical intakes and oversight of care plans, are provided by physicians at our affiliated medical practice. If you have questions about this policy, email hello@withnola.com.
What we collect
Information you give us. When you fill out a form on this site or email us, we collect what you choose to share. That usually includes:
- Your name, phone number, email address and your relationship to the person you're asking about.
- Information about the older adult: their name, contact details, where they live, and what's going on. This can include information about their health, insurance, benefits and care needs.
- For referrals, the referring person's name, organization and contact details, and whether the family has agreed to be contacted.
Information collected when you visit. Like most websites, the services that run this site receive basic technical information when you load a page, such as your IP address, browser type and the page you requested. We don't use analytics tools, advertising pixels or tracking cookies on this website. Our form provider may use cookies that its forms need in order to work.
How we use it
- To respond to your message or referral.
- To provide care management, including contacting the people and organizations who can help with what the family asked for.
- To bill Medicare and other insurers for covered care management services.
- To follow up with you about your request.
- To run and secure this website.
- To meet our legal obligations.
Who sees it
We share information only as needed to do the work you asked us to do, or as described here:
- The Nola team members working with the family.
- Our affiliated medical practice, whose physicians do clinical intakes and oversee care plans. Health information the practice receives as part of a patient's care is also subject to federal health privacy law (HIPAA).
- Medicare and other insurers, to bill for covered services.
- Health care providers, agencies, benefits offices, community organizations and service vendors, when we need to share details with them to arrange help the family asked for.
- The person or organization who referred the family. We may tell them whether we reached the family. We share more about the family's situation only with the family's agreement.
- Companies that run our tools, which handle information on our behalf: Jotform (our forms), Vercel (website hosting), Google (website fonts) and our email provider.
- When the law requires it, or when we believe sharing is needed to protect someone's safety.
We do not sell personal information, and we do not share it for advertising.
How long we keep it
We keep information while we're working with a family, and afterward for as long as we need it for our records or to meet legal obligations. Form submissions are stored with our form provider until we delete them. You can ask us to delete your information at any time (see below).
Your choices
You can ask us to:
- Tell you what information we have about you or your family member.
- Correct information that's wrong.
- Delete your information, unless we're required to keep it.
- Stop contacting you.
Email hello@withnola.com with your request. We aim to respond within 30 days, and we may need to confirm who you are first.
Keeping information safe
We take reasonable steps to protect the information we hold, but no website or system is completely secure. Please don't send urgent medical information through our forms or by email. In an emergency, call 911.
Children
This website is not directed to children under 13, and we don't knowingly collect information from them.
Do Not Track
Some browsers send a "Do Not Track" signal. Because we don't track visitors across other websites, our site doesn't respond differently to that signal.
Changes to this policy
If we change this policy, we'll post the new version on this page with a new effective date.
