Between doctor visits, everything lands on you. We take the to-do list.

Keep your doctor and family in the loop

What was done, what's next, and who to call.

Care works better when it's local. We know the senior centers, classes, faith communities and neighbors in your part of the city, so living at home doesn't mean being stuck there.

How it works

  1. 1

A free call to check your coverage

Tell us what's going on. We'll check your Medicare coverage on a quick call, so you know up front what to expect. Most people pay nothing.

  2. 2

    Meet your care team

Your care manager and a Nola clinician get to know you: your health, your history, your routines, and what matters most to you.

  3. 3

    Ongoing support, close to home

Your care manager builds a plan around how you want to live, handles the legwork, follows up until help arrives, and keeps you connected to your community.

A plan is the easy part. We make sure help arrives.

Care often stalls in the handoffs: the aide who doesn't show, the referral that goes nowhere. We follow through until help arrives, and keep you connected to the places that make your neighborhood home.

Common questions

What can Nola help me with?

Your care team handles the parts of healthcare that happen between doctor visits: benefits and insurance, paperwork, rides, food, home care, and keeping your doctors on the same page. We also connect you with senior centers, classes, faith communities and programs near you, so you stay active and connected. You get one team to call, and you stop being your own case manager.

How do I know if I qualify?

You likely qualify if you:

  1. Have Original Medicare (not Medicare Advantage)
  2. Live in the New York City area
  3. Have an ongoing health condition, or needs like food, housing or rides that get in the way of your care

Not sure? Book a free call and we'll check your eligibility, with no cost and no commitment. If Nola isn't the right fit, we'll point you to other resources that can help.

How much does Nola cost?

For most people, nothing. Nola is covered by Medicare. If you also have Medicaid, a Medicare Supplement (Medigap) plan or other secondary coverage, it typically pays the Part B coinsurance, so you pay $0 out of pocket. Without secondary coverage, you may owe a small coinsurance, like you would for a doctor's visit. We check your coverage on a free call before you start, so there are no surprises.

Does Nola replace my doctor?

No. You keep your own doctors and specialists. We work alongside them, keep everyone on the same page, and help make sure their instructions actually happen at home, from new prescriptions to follow-up appointments.

How often can I reach my care manager?

As often as you need. Call, text or email your care manager during business hours, and leave a message anytime. We also check in on our own, so you don't have to remember to call us.

Someone in your corner. So your time goes to what matters to you.
